- Please contact the HR Department to share with us the good news. HR will inform you about legal aspects that should be taken care of to start your Maternity leave (urlop macierzyński)
- You also should provide the HR with the copy of child's birth certificate. The documents should be submitted to the HR department once you have given birth to your child.
- Please remember that either you or the father of your child should report the new member of your family to ZUS. If you decide to do so - contact the HR department after obtaining your child's PESEL number and submit the form: Public Healthcare system for family members’ request.
Medical care
You may also request additional private medical care for your family. Aon covers this cost; you only need to cover the tax/insurance deductions and dental coverage for family members, if requested. To do so – please fill in the “Allianz partner family registration form”.
Please note, our colleagues who use unpaid parental leave (urlop wychowawczy) can continue to use the currently-used health care package till the end of the month following month when unpaid parental leave starts.
Starting the 3rd month of unpaid parental leave a colleague may use self-funded health care package. If you decide for such option please fill in a form: “Allianz registration request_parental leave”.
To sum up, documents that mothers need to deliver after the childbirth:
- Child's birth certificate (we will need a copy only).
- Maternity leave form (more details regarding maternity leave here).
- Registration to public healthcare system if you want to register baby in the Social Security via Aon: Registration Form
Please note: a child can be registered to public healthcare system by one of its parents only at the same time, but this is up to parents' decision which of them will apply for registration - Any insurance programme forms if applicable
